What makes Chroniq Health unique in the market?
There are a number of unique differences, but the one we are most proud of is that the platform was designed with real life workflows in mind.
We understand that chronic disease management is shared between the Patient, the Nurse and the GP, all playing an important role.
The patient is at the centre, with goals developed to support their individual care needs. The nurse has the flexibility to develop a care plan that reduces unnecessary time "ticking boxes" with more time to "be" with the patient, and the GP has a care plan that is patient focused, relevant, and Medicare compliant.
What is the biggest risk to the company?
Medicare funding is the ever present risk to general practice, and is a risk shared by many.
However, we feel confident that chronic disease management will be a continuing focus for Medicare and by developing a platform that is outcomes driven we will be well positioned to offer care well into the future.
